Registration Procedures - Forms Required

Application Form: Deadline March 31st, 2009

We are now accepting applications for the Flatgrass Regional Showdown online, with payment options for credit card and check. The fees for this year's tournament are $375 for U11 & U12 OR $425 for U13-U19. For proper seeding and quicker check-in, it is important that you complete the roster data and enter information on the results of your recent league and tournament games. This information can be updated until just before the tournament.

Additional Tournament Forms for All Teams:

Player Waiver Form: (Required at check-in) - Each player on the team must complete this form and acquire a parent sigature to be eligible to play in the tournament.
Coach and Manager Waiver - Each coach and manager should complete the waiver and present it at check in.

Guest Player Form: (Required at check-in) - If your team is using guest players there is a maximum of 4 guest players per team. This link will take you to the WYSA online guest player application. There is a cost of $5.

Additional Tournament Forms for Non-Wisconsin Teams:

Tournament Sanction Form: This form is used to help you complete the "Permission to Travel Form".

USSF Permission to Travel Form: (Required at check-in) - If your team is traveling to Wisconsin from out of state, please begin the process of acquiring permission to travel from your State Association. In some cases, it can take more than 30 days to acquire.
